The short answer

An FQHC is a health center that serves an underserved area or population, offers primary care regardless of ability to pay with a sliding fee discount, and is governed by a board on which most members are patients of the health center. It either receives a federal Section 330 grant from HRSA or is designated as a Look-Alike.

What an FQHC has to provide

HRSA’s Compliance Manual (Chapter 4) requires health centers to provide required primary health services directly or through arrangements, including preventive care and screenings, and enabling services such as case management and translation that help people use care. Preventive dental care is on the required list; many health centers also offer broader dental, behavioral health, and pharmacy services. Which services a particular site offers is not in HRSA’s public site file, so ask the site.

Who can use one

Anyone. Health centers see people with private insurance, Medicare, Medicaid, or no insurance. Patients with household income at or below 200% of the federal poverty guidelines are eligible for the sliding fee discount.

FQHC, community health center, and rural health clinic

“Community health center” is the everyday name for most FQHCs. A rural health clinic (RHC) is a different Medicare designation with its own rules and is not part of HRSA’s Health Center Program, so RHCs are not in this list. The CMS booklet in the sources explains how Medicare pays FQHCs.
